It combines two medications: phentermine, which suppresses appetite, and topiramate, which helps you feel full sooner and stay full longer. Neither is new. It is not a GLP-1 medication. It does not involve injections. If you have been researching Ozempic, Wegovy, Mounjaro, or Zepbound and decided that weekly shots are not for you, Qsymia is worth knowing about.

How Qsymia Works for Weight Loss
The two ingredients in Qsymia work through different pathways, which is part of why the combination is effective. Phentermine acts on the central nervous system to reduce hunger signals. Topiramate, originally developed for seizures and migraines, appears to reduce appetite and increase the feeling of fullness through mechanisms that are still being studied but are well-documented in practice.
Before you come
The result: you feel less hungry, you feel satisfied with less food, and the urge to eat between meals tends to quiet down. This is different from how GLP-1 medications like Wegovy or Zepbound work, which primarily slow digestion and affect gut hormones. Neither approach is universally better. Who Is a Candidate for Qsymia?
Qsymia is FDA-approved for adults with a BMI of 30 or higher, or a BMI of 27 or higher with a weight-related health condition such as high blood pressure, type 2 diabetes, or high cholesterol. That said, Dr. Gougian does not treat eligibility as a rigid checklist. The initial visit is a conversation about your health history, your goals, and what has and has not worked before.
There are some situations where Qsymia is not appropriate. Qsymia should not be taken during pregnancy and requires a monthly pregnancy test for women who could become pregnant, this is an FDA requirement. It is also not recommended for people with glaucoma, hyperthyroidism, or certain heart conditions, or for those taking MAO inhibitors. If you have a history of kidney stones, that is worth mentioning at your visit as well.
If Qsymia turns out not to be the right fit, there are other options. Phentermine alone, GLP-1 medications, and other approaches are all on the table. The goal is finding what will actually work for you.
