Meet Dr. Rachel Gougian, DO, FAAFP, D-ABOM

  • Diplomate, American Board of Obesity Medicine
  • USCIS-designated Civil Surgeon
  • Fellow, American Academy of Family Physicians

Dr. Rachel Gougian is the founder of Pandora Family Medicine and the physician you’ll see when you establish care here. She is an osteopathic family physician, a Fellow of the American Academy of Family Physicians, and a Diplomate of the American Board of Obesity Medicine. She is also a USCIS-designated Civil Surgeon who has completed thousands of immigration medical exams.

Before medical school she studied health policy at Carnegie Mellon University, and it shows in how the practice runs. She thinks about who can actually get care, what it costs, and how to make follow-up simple, not only about what to prescribe. She sees patients in English and Spanish.

Clinical Interests & Expertise

Three areas of depth, practiced side by side in one small office.

Obesity medicine and physician-guided weight loss

Board certification in obesity medicine means weight is treated as a medical condition, not a character flaw. Dr. Gougian prescribes Wegovy, Zepbound, Ozempic, Mounjaro, phentermine, Qsymia, and other options when they fit, handles prior authorization, and follows you closely. $200 for the first visit, $150 for follow-ups, no subscription.

Weight loss services

Immigration medicine

As a Civil Surgeon she completes the I-693 for green card applicants from across the Washington, DC area. Immunity testing means you only get the vaccines you actually need, and the sealed envelope is ready on a timeline you can plan around.

Immigration medical exam

Family medicine, skin care, and aesthetics

Checkups, chronic conditions, mental health, in-office procedures, and the aesthetic treatments she uses herself: Botox, Lumecca IPL, and tretinoin. Medicare and Medicaid patients are welcome.

Why She Founded Pandora Family Medicine

Dr. Gougian opened Pandora Family Medicine because she wanted to practice the kind of medicine that is getting harder to find: one doctor, who knows you, with enough time to listen. In a large system you can see a different clinician at every visit and repeat your story each time. Here, you establish care with one physician, and that relationship is the point. In her words, Pandora Family Medicine is a place where a patient can establish care with one doctor and feel like they are known.

The aesthetics side of the practice started, as she puts it, as a selfish pursuit. She wanted IPL for her own skin, tretinoin that actually worked, and Botox from someone she trusted, so she trained in them and now offers the same treatments to patients. There is no upsell, just a doctor recommending what she uses herself.

The practice was built to grow. Over time Pandora Family Medicine expects to welcome two or three physicians who share the same approach, so more neighbors can have a doctor who knows their name. The commitment stays the same: same doctor every visit, honest pricing, and nobody made to feel rushed.
