What Is Phentermine?
Phentermine is a prescription appetite suppressant that has been FDA-approved for weight loss since 1959. It is one of the most prescribed weight loss medications in the United States. If you are skeptical of newer medications and want something with decades of real-world use behind it, phentermine has that track record.
It is also a Schedule IV controlled substance. That is worth saying plainly, because patients who research phentermine already know it. Schedule IV means it has recognized medical use and a lower potential for dependence than higher-schedule drugs, but it does require a prescription, and refills are handled differently than a standard medication. We will walk you through what that means in practice.
Phentermine is typically prescribed for short-term use, often 12 weeks at a time. It works best as part of a broader plan, not as a standalone fix. If you need longer-term support after phentermine, we offer other options including GLP-1 medications like Ozempic, Wegovy, Mounjaro, and Zepbound, and combination medications like Qsymia.
How Phentermine Supports Weight Loss
Phentermine works primarily by reducing appetite. It stimulates the release of norepinephrine in the brain, which signals your body that it is not hungry. For many patients, this makes it easier to eat less without feeling deprived or fighting constant cravings.
Before you come
It is not a passive medication. Phentermine works best when paired with changes to how you eat and move. The appetite suppression gives you a window to build those habits without hunger working against you the entire time.
The visit
Because it is a Schedule IV medication, refills require an in-person or telehealth visit rather than automatic renewals. We schedule follow-up appointments to monitor how you are responding, adjust the dose if needed, and make sure the medication is still the right fit.
Vaccines, only the ones you need
We compare your records against the USCIS vaccine list. Where a record exists, you are done. Where one is missing, immunity testing can usually show you are already protected, so you don’t repeat shots you have already had.
Results and the sealed envelope
When lab results are back, Dr. Gougian completes and signs the I-693 and seals it in an envelope for USCIS. With complete records and normal labs, that is often within a few days of your visit. If a vaccine dose or repeat test is needed, we give you a realistic date before you leave. Don’t open the sealed envelope; you receive a separate copy to keep.
If something turns up
A positive tuberculosis blood test, for example, usually means a chest X-ray, not a denial. We explain what it means, arrange what is needed, and complete the form when it is done. You are never left guessing.
We know the timeline matters. Many patients are working against an interview date or a request for evidence, and we schedule and process exams with that in mind. The exam is a flat fee quoted before you book; blood tests and vaccines are itemized and added only if you need them, so there are no surprise costs.
Who Qualifies for Phentermine?
Phentermine is FDA-approved for adults with a BMI of 30 or higher, or a BMI of 27 or higher with a weight-related health condition such as high blood pressure, type 2 diabetes, or high cholesterol. If you are not sure whether that includes you, just ask. We are not here to gatekeep.
There are some medical situations where phentermine is not appropriate, including uncontrolled high blood pressure, certain heart conditions, hyperthyroidism, a history of stimulant sensitivity, and pregnancy. Dr. Gougian reviews your full history at the initial visit to make sure phentermine is a safe choice for you specifically.
You do not need to prove you have tried hard enough or failed enough times. If phentermine fits your health picture and your goals, that is the conversation we have.
Phentermine is not right for everyone, and if it is not the best fit, there are other options. Wegovy, Zepbound, Mounjaro, and Qsymia are all part of the weight loss program at Pandora Family Medicine. Dr. Gougian can help you think through which medication makes the most sense given your history, your health, and what you have tried before.
Schedule OnlinePhentermine Safety: What Patients Ask Most
Phentermine has been studied and prescribed for over 60 years. That history is actually one of its strengths. Common side effects include dry mouth, increased heart rate, difficulty sleeping, and decreased appetite, which is the point. These are generally manageable and often ease after the first few weeks.
Serious side effects are uncommon but real. Phentermine is not recommended for people with uncontrolled hypertension or a history of heart disease, which is why an in-person evaluation matters before starting. We check your blood pressure and review your history at every visit.

The initial weight loss visit is $200. Follow-up visits are $150. There is no subscription, no monthly program fee, and no package you have to buy. You pay for the visit, and we figure out the medication together.
Many telehealth weight loss services like Ro, Noom, and Calibrate charge ongoing subscription fees regardless of whether you are actively using the program. We do not work that way. You come in, we talk, and you only pay for the care you actually receive.
