Prior Authorization for Weight Loss Medication Shouldn't Fall on You

Prior authorization, or PA, is a requirement from your insurance company. Before they'll cover certain medications, they want documentation showing that the prescription is medically appropriate for you. For weight loss medications, this almost always applies.

When PA is approved, your insurance covers the medication at your normal cost-sharing rate. Without it, you're paying full price, which for GLP-1 medications like Ozempic, Wegovy, Mounjaro, or Zepbound can run several hundred dollars a month or more.

The frustrating part is that insurers don't make the process obvious. Requirements vary by plan. Some want lab results. Some want documentation of prior weight loss attempts. Some want a specific diagnosis code. Getting it wrong means starting over.

   GLP-1 medications

   Wegovy and Zepbound are FDA-approved specifically for weight loss, which helps with coverage, but PA is still typically required. Ozempic and Mounjaro are approved for type 2 diabetes, and coverage for weight loss use varies significantly by plan.

   Older medications

   Phentermine and Qsymia tend to have lower price tags, but some plans still require PA, and coverage for weight management medications is inconsistent across insurers. We'll look at your specific plan and tell you what to expect before we submit anything.

   Vaccines, only the ones you need

   We compare your records against the USCIS vaccine list. Where a record exists, you are done. Where one is missing, immunity testing can usually show you are already protected, so you don’t repeat shots you have already had.

   Results and the sealed envelope

   When lab results are back, Dr. Gougian completes and signs the I-693 and seals it in an envelope for USCIS. With complete records and normal labs, that is often within a few days of your visit. If a vaccine dose or repeat test is needed, we give you a realistic date before you leave. Don’t open the sealed envelope; you receive a separate copy to keep.

   If something turns up

   A positive tuberculosis blood test, for example, usually means a chest X-ray, not a denial. We explain what it means, arrange what is needed, and complete the form when it is done. You are never left guessing.

We know the timeline matters. Many patients are working against an interview date or a request for evidence, and we schedule and process exams with that in mind. The exam is a flat fee quoted before you book; blood tests and vaccines are itemized and added only if you need them, so there are no surprise costs.

PA assistance is part of what we do here, not an add-on service with a separate fee. When you're a patient at this practice, your visit includes the documentation work that PA requires.

That means we pull together your medical history, weight history, relevant lab results, and diagnosis codes in the format your insurer is looking for. We submit the PA request. If your insurer asks for more information, we respond. If they deny the request, we don't just hand that back to you.

We file appeals. When it's appropriate, Dr. Gougian conducts peer-to-peer reviews, which means she speaks directly with the insurance company's medical reviewer to make the case for your coverage. That's something a telehealth app or a prescription-only service isn't going to do for you.

It depends on your insurer. Some plans respond within a few business days. Others take two to three weeks. We submit requests promptly and follow up if we don't hear back. We'll give you a realistic timeline based on what we know about your plan.

We file an appeal on your behalf. If that doesn't resolve it, Dr. Gougian can request a peer-to-peer review with the insurance company's medical reviewer. If the appeal still doesn't go through, we'll talk about what other options make sense for you, whether that's a different covered medication or cash-pay pricing.
